I realise that you may not have received a reply re the V55/5. Here's what you need to complete (I've used my DT3 details as an example, where appropriate):-

1 (Country purchased from) - YOURS 
2 (Tax Class) - Historic Vehicle
6 (Make) - Yamaha
7 (Model) - DT3
8 (Type of body) - Motorcycle
9 (Wheelplan) - Two wheel
10 (Colour) - YOURS
27 (Year of manufacture) - YOURS
31 (Type of fuel) - Petrol
32 (VIN) - YOURS
33 (Engine number) - YOURS
34 (Cylinder capacity) - YOURS
63 (Details) - YOURS
64 (Details) - YOURS
65 (Exemption) - Exempt by age
66 (Mileage) - YOURS

Apart from signing and dating it, everything else is left blank.

I sent this to DVLA with my cheque, dating certificate (I used the Aircooledrd officer, but any approved club will do, or send the US title if you have the original document (it will be retained by DVLA)), printout of NOVA details (belt and braces, as DVLA can probably access the NOVA database), copy of my driving licence (for id purposes) and form V112 (might not be necessary).

DVLA might request an inspection of the vehicle. This is contracted out to a third party and the chap who inspected mine worked, I think, for a loss adjusting company.

ive looked on air cooled rd club and they say they can do RDs only so I’ll have to
use another service (VJMCC?)

I’ll need to sort out some photos first

Who did you speak to, Paul Dawkins? Paul did all of my non RD Yamahas for me, although they are more expensive than those for RDs.

Franky's posts and communications gave me a kick up the jacksy to get my own DT360A registered. Our DVLA sent me a log book (Title in US language) within 10 days of me posting the forms to them.
